This Hospice provides specialist palliative care, education, support and advice across Hawke's Bay helping people make the most of living and dying within a compassionate community.

We're seeking a Social Worker to join the Family Support Team, working 20 hours per week initially, with the opportunity to increase as service demand continues to grow.

The opportunity


This role will suit a social worker who is confident, efficient and thoughtful in their practice.

Someone who can deliver high quality support while maintaining realistic service boundaries in a growing service.

You’ll work alongside two part time social workers, helping cover key service gaps (particularly Mondays and Fridays) and contribute to a sustainable, responsive model of care.

What you'll be doing

You'll provide effective social work services to patients and their family/whānau, and contribute as a valued member of the interdisciplinary team.

Key responsibilities include:

  • Psychosocial assessments, care planning and targeted interventions
  • Discharge planning and mobilising supports promptly
  • Active participation in MDT meetings across inpatient and community settings
  • Family meetings, advocacy, and coordination with community partners
  • Accurate documentation and management of patient information in PalCare
  • Supporting grief and bereavement services, including staff debriefing as appropriate

What we're looking for

  • Current NZ Social Work Registration
  • Minimum 4 years social work experience ideally from within health
  • Confidence working within a multidisciplinary health team and advocating professionally
  • Experience in aged care and / or existing relationships and confidence working with ARC providers and community service
  • The ability to complete NASC assessments is an advantage
  • Full, clean NZ driver's licence and being comfortable with travel across the region, including Central Hawke's Bay (fleet car provided)
