This employer is committed to improving health outcomes for their communities through effective primary health care being one of the largest primary health organisations (PHO) in the Wellington region, incorporating Wellington City, Porirua/Kapiti and the Wairarapa. They exist to enable better health through great primary care. They are a registered charitable trust, which provides primary health care and community health services to an enrolled population of around 315,000, mainly through a network of general practice and employed staff. They have offices in Masterton, Porirua and Wellington.

About the role

  • The core function of this role is to empower and navigate clients and fanau to improve overall health and well being outcomes for the Pacific community in the CCDHB District
  • Being part of the Pacific Navigation service team, which has staff located in Wellington and Porirua
  • Being a key contact person between Pacific fanau, general practice, primary health care teams, the Pacific community and other stakeholders
  • Working with primary and secondary health care providers to identify the priority people who are not accessing primary health care at levels appropriate for their health need and support these whanau to get health care

About you

  • Experienced working with Pacific families
  • Passionate about being part of a team to influence positive health change within Pacific families
  • Fluent in a Pacific language
  • Excellent interpersonal and communication skills
  • Experience in the health sector, including working with a clinical workforce
