About the Job
- As a Nurse Practitioner you will work autonomously, but also in conjunction with an integrated and multidisciplinary team to deliver quality evidence based primary care services to your own patient load (the service has a total enrolled patient population of around 6,500)
- There is a broader approach here to improving family and community well being. You’ll be able to easily refer to other services on site by simply heading down the corridor together
- You will hold appointments, conduct physical assessments, diagnose and treat common conditions, provide screenings and prescribe medications. Oh yes, and there is always the paperwork!
- There is huge diversity of work here, you’ll deal with whatever walks in the door, but also allowing for special interests. All clients are triaged first to ensure everyone is working at the top of their scope.
- You’ll work in a patient centred medical home model. This includes GP, RN, Nurse Prescribers, Admin, HCA’s, Health coach, HIP and clinical pharmacist. What a great team around you to work alongside!
- You will rotate through the Respiratory and Long term conditions clinics.
- Full Time, Monday to Friday.
About you
- Prior experience in a primary care setting is ideal. However we will consider other clinical experience
- Keen on working in a Kaupapa Māori organisation. You’ll be committed to learning Tikanga and Te Reo Māori
- A wonderful communicator – able to develop trust and relationships with a holistic / whānau focus.
- Committed to continual improvement. This organisation is genuinely open to new ideas on how to work better
- You’ll enjoy autonomy and working independently but naturally want to contribute to the team and work closely with others
- Agile, adaptable and flexible with a can do attitude. You’ll find that no two days are the same
- Well organised with excellent time management skills
- You must be fully vaccinated and able to be fit tested for full PPE gear when supporting the respiratory clinic and/or working with patients at risk.
