About the Role
- Provide hospital-level clinical care and support patients working towards their goals in community living to achieve positive outcomes.
- Work holistically with your clients - overseeing clinical care, completing assessments, doing medication rounds, and delegating to Support Workers.

About You
- Enrolled Nurse with a current practicing certificate.
- Strong organisational and leadership skills.
- Able to communicate effectively with other health professionals and within a multi disciplinary team.
- Excellent documentation abilities.
- Ability to engage and connect with a diverse range of people.
- Previous clinical knowledge and practice within disability or rehabilitative services is preferred but not essential.
