Acute Care Physical Therapist

Overview: Assess and treat patients admitted to the hospital to maximise their independence once they are discharged

• Evaluate patients with a wide variety of diagnoses in a level 1 trauma centre 

• Design and implement treatment plans tailored to address each patient’s needs
• Collaborate with rehabilitation and medical teams to ensure patient safety and progress within the care plan
• Coordinate and communicate with family members and caregivers to facilitate effective treatment and provide education to patients and families appropriate to their level of understanding and motivation

Overview: Assisted patients recently discharged from the hospital in returning to their prior level of function

• Performed physical evaluation to determine treatment and assessed patient’s needs to establish if they were appropriate referrals for other home health services such as nursing, occupational therapy, speech language therapy, social work
• Performed OASIS assessments in accordance with CMS guidance 

• Supervised team of Physical Therapy Assistants in the implementation of care plans
